Hospice Care

Liberty Lutheran, the trusted name in hospice care
in Pennsylvania.

Liberty Hospice supports patients and families facing a life-limiting illness. Typically, hospice services are available to patients with a life expectancy of six months or less if the disease runs its normal course. Hospice eligibility needs to be certified by a physician, most often the patient’s own physician, in conjunction with the hospice medical director.

Our multi-disciplinary care team works with physicians, care providers and families to assure quality of life for patients facing life-limiting illnesses. Liberty Hospice respects and responds to the unique differences in lifestyle, values and wishes of the patients, their families and caregivers.

At Liberty Hospice, our chaplains and social workers provide spiritual, practical and social support, as well as empathic listening for patients and family members. Liberty Hospice reflects a philosophy of care and compassion that helps people experience their end-of-life journey with comfort and dignity.
